Service Description

Supports clients with cognitive impairments with funding of $500/month to purchase home support services (personal care, respite, meal preparation, and household chores).

Under this program, a person may also be eligible to receive reimbursement for snow removal services, up to a maximum of $495/year.

Eligibility

Individuals must be:
* Residents of Nova Scotia with a valid Health Card
* Have significant memory loss and memory problems that affect daily functioning
* Be deemed by Continuing Care as needing a minimum of 25 hours/month of care support
* Have a Substitute Decision Maker (someone who will act on your behalf and has signed an agreement that defines terms and conditions for this program)

Application Process:
Call the toll free number. An Intake Worker will document basic information about you and forward this information to a Care Coordinator. A Care Coordinator will call you back and arrange to meet with you. During this meeting, you will be assessed regarding your care needs and the Care Coordinator will talk to you about the outcome of the assessment.

If you are already a client of Continuing Care, but feel your needs have changed, contact your care coordinator.
