- Description
Provides individualized funding to participants living at home with their families or who live independently with support from their families or personal support networks.
The funding is used to:- Purchase supports specific to a participant's disability-related needs and goals
- Promote the participant's independence, self-reliance, and social inclusion
- Offer an alternative to, prevent, or delay a participant's placement in a DSP-funded residential support option
- Eligibility
- To be eligible, the applicant and their family must meet the following criteria:
* The applicant and their family are permanent residents of Nova Scotia
* Have a valid Nova Scotia health card
* The applicant is 19 years or older
* The applicant has a medical diagnosis of 1 or more of the following: Intellectual disability, long term mental illness, or physical disability. Persons with an acquired brain injury, which results in damage to a person's brain that occurs from events after birth rather than as part of a genetic or congenital disorder, may also be included in 1 of the above 3 categories depending on their functional assessment.
* The applicant and family have unmet needs as identified through the assessment process
* The applicant meets the financial eligibility criteria as outlined in the Disability Support Program Policy
* The applicant and their family agree to participate in the assessment process as outlined in the Disability Support Program Policy - Application Process
